Definition

What is Shiatsu Massage?

Shiatsu is a Japanese bodywork tradition that grew out of anma massage and Western anatomy training in the early 1900s, formally codified by Tokujiro Namikoshi and later expanded into the meridian-focused style most American studios practice today. The therapist uses thumbs, palms, and elbows to apply sustained perpendicular pressure to tsubo points along the body's meridian lines — energy pathways mapped by traditional Chinese medicine. You stay fully clothed under a blanket; there is no oil and no kneading. Sessions feel less like spa pampering and more like targeted release: tight neck and shoulder muscles soften, breathing deepens, the parasympathetic nervous system shifts dominant. At Healing Shiatsu on Balboa St in San Francisco's Outer Richmond, our licensed therapists bring years of experience to every session, which run 30 to 120 minutes from $65, with walk-ins welcome seven days a week.

The meridian and Qi framework behind shiatsu comes from traditional Chinese medicine, described by NCCIH as a system that has evolved over thousands of years. We present that tradition as tradition. On the modern-evidence side, reviews summarized by NCCIH suggest massage may provide short-term relief for neck, shoulder, and muscle tension and may help ease anxiety — honest, measured benefits rather than cures.

The Science & Tradition

How Does Shiatsu Work?

1

Meridian-Based Healing

Shiatsu is based on the holistic system of traditional Chinese medicine, where illness is thought to result from imbalances in the natural flow of energy, or Qi, through the body.

2

Finger & Palm Pressure

Shiatsu therapists use finger and palm pressure to energetic pathways called meridians to improve the flow of Qi throughout the body.

3

Nervous System Balance

A common modern explanation is that sustained pressure helps shift the body away from an over-activated sympathetic “fight-or-flight” state. Research reviewed by NCCIH suggests massage may offer short-term relief for muscle tension, neck and shoulder discomfort, and anxiety — effects many clients describe after a session.

4

No Oil Required

During a Shiatsu session, you are under a blanket and no oil or lotion is used. After the massage, natural lotion is available upon request.

The Experience

What Does Shiatsu Feel Like?

The shiatsu therapist applies pressure using fingers, thumbs, and palms in a continuous rhythmic sequence.

The pressure feels more localized — finger pads are used instead of the entire palm for most of the treatment.

Certain pressure points may feel tender, which some people describe as "good pain."

If you feel any discomfort during treatment, tell your therapist and they will adjust the pressure to your comfort level.

When Shiatsu Helps Most

Conditions Shiatsu Commonly Addresses

Shiatsu is most useful for everyday tension patterns rather than acute medical problems. For tension-type headaches, Mayo Clinic notes that gentle massage of the temples, neck, and shoulders can help relieve the muscle tension that often triggers them. For sleep, NCCIH describes relaxation-based approaches as being studied for insomnia, while Mayo Clinic points to massage as one supportive way to lower stress and ease anxiety. None of this replaces medical care — it works alongside it.

  • Tension headaches — the band-around-the-head type
  • Cervical strain from desk work and screen time
  • Frozen shoulder in early or maintenance stages
  • Sciatica radiating from the lower back into the glutes
  • Insomnia tied to nervous system overactivation
  • Anxiety with somatic symptoms — jaw clenching, chest tightness
  • TMJ dysfunction and jaw tension
  • Sports recovery — climbing, cycling, running

Honest limits.

Shiatsu is not a replacement for physical therapy or medical care for acute injuries. It works best for chronic, low-grade tension patterns that respond to sustained pressure and nervous system down-regulation. If you have a herniated disc, recent surgery, a fracture, or active inflammation, talk to your doctor or PT first — we'll work alongside their plan, not around it.

Before Your Visit

How to Prepare for Your Shiatsu Session

  1. 1

    Eat lightly an hour beforehand

A full stomach makes lying face-down uncomfortable. A snack is fine; a full meal isn't.

  2. 2

    Wear loose, comfortable clothing

Yoga pants, sweatpants, a soft t-shirt. Avoid jeans, belts, or anything restrictive — the therapist works through the fabric.

  3. 3

Skip caffeine two to three hours before

Caffeine keeps the sympathetic nervous system activated, which is what Shiatsu is trying to calm. If you can't skip it, the relaxation depth will just be lighter.

  4. 4

Tell your therapist what's going on

Recent injuries, medications (especially blood thinners), pregnancy status, areas to focus on, areas to avoid. The more we know, the better the session adapts.

  5. 5

    Hydrate before and after

Hydrated muscle tissue responds better to pressure. Drink a glass of water before the session and another within the hour after.

What is Shiatsu massage?
Shiatsu is a traditional Japanese massage technique that uses finger and palm pressure along the body's meridians (energy pathways) to improve the flow of Qi. It is based on the holistic system of traditional Chinese medicine.
Do I need to undress for Shiatsu?
No. Shiatsu massage is performed while you are fully clothed under a blanket. No oil or lotion is used during the session, though we offer natural lotion afterward if you'd like.
Is Shiatsu massage painful?
Shiatsu pressure can feel intense at certain points — some people describe it as "good pain." However, if you feel any discomfort, tell your therapist and they will adjust the pressure to your comfort level.
What are the benefits of Shiatsu?
Shiatsu helps release stress, relax the neck, shoulders and back muscles, improve circulation, relieve stiff muscles, and restore the natural flow of energy through your body.
How long is a typical Shiatsu session?
Sessions are available in 30, 45, 60, 75, 90, and 120 minute durations. Your therapist will discuss the best option based on your needs.
How is Shiatsu different from Swedish massage?
Shiatsu uses finger and palm pressure on specific meridian points without oil, while Swedish massage uses long flowing strokes with oil. Shiatsu focuses on energy pathways, making the pressure feel more localized and targeted.
